Dear Tree Warden,
I have a large tree on the grass strip between the sidewalk and street in front of my home. It does not appear healthy and is starting to present a concern to me and my neighbors. During the storm two weeks ago it lost a branch that could have caused damage if a car were parked underneath it. I also see some branches that are missing the bark and I am afraid it will hurt someone and/or damage my home.
I am requesting that someone come out to evaluate the health of the tree and/or remove at least the dead branches on the tree.

A large dead branch fell off of the street tree outside of 34 Humiston Ave. The tree has lost multiple large branches over the past two years and does not appear in good health. The tree also appears to have an insect infestation at its base. Please send a representative to check the health of the tree.
